He just died... Yeah he did, he didn't know how he died but the feeling is there the feeling of pain was like worms crawling all over his skin. He knows that he was reincarnated or to be more precisely transmigrated with... a cliche name in that. He doesn't remember anything well he does remember the things he watched, read, and played but the memories of who he was and why he did where not there... it's creepy.
