The hospital never truly slept.

Even in the quieter hours of the afternoon, when the sunlight filtered softly through the high windows and the usual rush settled into a steady rhythm, there was always movement—monitors beeping, footsteps echoing, low conversations weaving through sterile corridors. It was a place built on urgency disguised as order.

Alexander Kazafi thrived in that order.

Routine grounded him. Precision defined him. Every patient, every diagnosis, every decision followed a structure he trusted completely.

And that was exactly why this—whatever this was—felt like a problem.

Not because of the disruption.

But because of the unpredictability.

"Doctor Kazafi."

A nurse approached him with hurried steps, her expression tight with unease. "Room 318… the patient is refusing treatment again. He's getting aggressive."

Alexander didn't pause in his stride. "Vitals?"

"Stable, but his blood pressure is rising. He's agitated—he won't let anyone near him."

Alexander gave a short nod. "I'll handle it."

Room 318 wasn't far, but as he approached, he could already hear raised voices from inside. A man shouting. Something metallic hitting the floor. The sharp edge of fear in a nurse's attempt to calm him down.

Alexander pushed the door open without hesitation.

Inside, chaos.

The patient—a man in his early forties, broad-shouldered but clearly weakened—was half out of bed, ripping at the IV line in his arm. A tray had been knocked over, instruments scattered across the floor. Two nurses stood at a distance, uncertain, trying to de-escalate without getting too close.

"Get away from me!" the man snapped, breathing heavily. "I said I'm fine!"

"You're not fine," Alexander said calmly, stepping inside and closing the door behind him.

The man's head snapped toward him. "Another doctor? Great. Come to lecture me too?"

Alexander didn't react to the tone. He walked forward, steady and controlled, his gaze sharp but not confrontational.

"No," he said. "I came to treat you."

"I don't need treatment!"

"You do."

The man let out a harsh laugh. "You all say that. You don't even listen."

Alexander stopped a few steps away, close enough to engage but not close enough to threaten.

"Then talk," he said simply.

That caught the man off guard.

"…What?"

"Talk," Alexander repeated. "You don't want treatment. Why?"

The man hesitated, clearly not expecting the question. His anger faltered for a moment, replaced by confusion.

"I—… it's nothing. I just want to leave."

"You can't."

"Watch me."

"You'll collapse before you reach the exit," Alexander replied evenly. "And if you're lucky, you'll be brought back here unconscious instead of somewhere worse."

The bluntness landed.

The man's jaw tightened. "You think I'm scared?"

"No," Alexander said. "I think you're reckless."

Silence stretched between them, tension thick but no longer explosive.

Alexander took another step forward.

"Sit down," he said.

For a moment, it looked like the man might argue again.

Then, slowly—reluctantly—he did.

It was controlled. Managed. Exactly how Alexander handled situations like this.

Until—

The door opened.

---

Alexander didn't need to turn to know who it was.

The shift in the air was enough.

Heavy. Commanding. Unmistakable.

Vladimir Volkov stepped inside.

The patient noticed him immediately. His posture stiffened, eyes flicking over Vladimir's towering frame, the tattoos visible beneath the open collar of his shirt, the sheer physical presence that seemed to fill the entire room.

"…Who the hell is that?" the man muttered.

Alexander's expression hardened slightly.

"You shouldn't be here," he said without turning.

Vladimir didn't respond right away.

His gaze moved from the patient—

To Alexander.

"You seemed occupied," he said calmly.

---

That was not an answer.

---

Alexander finally turned, his eyes sharp. "This is a restricted area."

"And yet," Vladimir replied, stepping further inside, "I'm here."

The tension shifted.

Not gone—

But redirected.

---

The patient glanced between them, unease creeping into his expression. "Look, I don't know what's going on here, but—"

"Quiet," Vladimir said.

The word wasn't loud.

But it carried weight.

The patient fell silent instantly.

---

Alexander's jaw tightened. "You don't get to—"

"He's escalating," Vladimir interrupted, his tone still calm, almost detached. "His breathing is uneven, his pulse is unstable, and he's one step away from ripping that line out again."

Alexander paused.

Because—

He wasn't wrong.

"I'm handling it," Alexander said.

Vladimir's gaze returned to him.

"…Are you?"

It wasn't a challenge.

Not exactly.

But it wasn't neutral either.

Alexander stepped closer, lowering his voice. "Leave."

Vladimir didn't move.

---

For a brief moment, the two of them stood there—too close, too aware, the air between them tight with something unspoken.

The patient shifted suddenly, wincing, his hand tightening around the IV line again.

Alexander turned immediately, refocusing.

"Don't," he said sharply.

---

But the man was panicking again, breath coming faster, movements more erratic.

"I—I can't—this—" he stammered, clearly spiraling.

Alexander moved forward to stabilize him—

But before he could—

Vladimir stepped in.

---

It was quick.

Controlled.

Efficient.

He caught the man's wrist—not violently, but firmly enough to stop the movement entirely.

"Stop," he said.

The patient froze.

---

Not because of force.

But because of presence.

Vladimir's grip was steady, unyielding, his gaze locked onto the man's.

"Breathe," he said.

The word carried a strange kind of authority.

Not medical.

But absolute.

"…In," Vladimir instructed.

The man obeyed.

"…Out."

The room went quiet.

Alexander watched.

Within seconds, the patient's breathing began to slow. The panic receded, replaced by a stunned, almost disoriented calm.

His movements were precise as always—but his mind wasn't entirely on the task.

Then turned.

"You're done here," he said.

Vladimir met his gaze.

"…Am I?"

"Yes."

"Fine," he said.

"…Focus on your recovery," he said finally, his tone returning to its usual calm.
