Isane was one of the few people who shared a truly close and intimate bond with Unohana. She was aware of the scar on Unohana's chest—usually hidden by her braid—because she had seen Unohana with her hair undone.
Isane Kotetsu was primarily a medical apprentice and support specialist. She was an expert in Reiatsu Restoration. She could use her energy to replenish the depleted spiritual power of others. Her medical expertise was exceptional.

At first, I thought it was a fluke when I read his file and questioned why he was allowed to be an examinee. But he was permitted because Nemuri Kayama—Midnight-sensei—is his guardian."
She hesitated before asking softly, "But why do you need this, Mother?"
Unohana was a mother-figure to Isane. After all, she had adopted her. Unohana had found Isane when she was lost and didn't know what to do. Since then, Isane had become the person she cared for most in her current life.
Unohana did not truly want to be called "Mother," but she had grown accustomed to it.
Unohana opened the folder. "I have something to check," she said calmly.
Isane did not question further. She stood beside Unohana quietly, reading the information as well. It barely contained anything—just Maki's name, where he lived, and who his guardian was.
Unohana raised her head. "There is no past record about him?"
Isane tilted her head slightly. "There's nothing. Maybe we should ask Midnight-sensei."
Unohana closed the folder.
She smiled.
"No need, Isane. I'll take my time unraveling this mystery myself."
Isane looked at her mother and gulped at that smile.
"Mother… don't tell me you're into him or something."
"H-he's more younger —than y-you… I-is he going to be my stepfather soon?"
Just thinking about it gave Isane goosebumps—a young father?
Unohana raised an eyebrow and smiled genuinely.
"Well, as long as he can satisfy me and my—"
She did not continue.
But to Isane, hearing her mother speak like that felt like the world was crumbling to pieces. Her mother, who had rejected rich and powerful men who proposed to her, was now interested in some youngster?
Then what did she mean by "satisfy you—"?
Many questions and inappropriate scenarios ran through Isane's mind.

the idea of this shit came from the original details of how Shinigami have long life.
In the Bleach universe, the longevity of a Shinigami is tied to the fundamental laws of the afterlife and their own internal power. While a human might live 80 to 100 years, a Shinigami like Unohana can live for over 1,000 years while still appearing to be in her prime
And the idea of the percent of Shinigami that I put her was all based by the thing called Reishi and Reiryoku of bleach anime.
Composition of Reishi (Spirit Particles)
Unlike humans made of biological cells and atoms, Shinigami are made entirely of Reishi.
Slow Aging: Reishi-based bodies age at a significantly slower rate than Kishi (material) bodies.
The Environment: The Soul Society is a world composed entirely of Reishi. This environment naturally sustains spiritual beings, slowing the "decay" that we associate with aging
Reiryoku (Spiritual Power)
A Shinigami's lifespan is directly linked to their Reiryoku levels.
The "Engine" Effect: Spiritual power acts like a fuel that maintains the integrity of their Reishi body. The stronger a Shinigami's spiritual pressure (Reiatsu), the slower they age.
