Chapter 389 - Chapter 389: A Peril Over Pakistan, A Bad Attitude, And A Stormy Cockpit!

"Hello there viewers...Keiko here again...*Keiko proceeds to go and say this while traditionally bowing to the viewers as usual. To which Keiko then proceeds to go and bring her attention and eyesight back up again, in order to go and continue to address the viewers. Keiko also now proceeds to go and briefly clear her throat, as Keiko then proceeds to go and continue, to properly address the viewers*...

Now then viewers, for this chapter I will once again be covering several more transportation related accidents in history...And for this particular chapter viewers as like the last one, we will be covering another three of them...

And each of them, as shown by the chapter title of this particular chapter, is going to be covering three aviation accidents, all of which were attributed in some way, to a case of 'poor cockpit resource management'...

Now then viewers, lets begin with the first of the three aviation accidents...

And for this one, we will start by going all the way back, to July 28, 2010...

And on this particular day...Air Blue Flight 202...Which was a scheduled domestic passenger flight from Karachi, Pakistan, to Islamabad, Pakistan. And on this particular day, the aircraft that was being used to operate the flight, was an Airbus A321-231...

And on board Flight 202, were two pilots, 4 flight attendants, and 146 passengers... 

The captain of Flight 202, was 61-year-old Pervez-Iqbal Chaudhry, who had 25,497 hours of flying experience, with 1,060 hours on the A320 aircraft...

The first officer, was 34-year-old Muntajib-ud-Din Ahmed, a former F-16 Pakistan Air Force fighter pilot and squadron leader, who had 1,837 hours of flying experience and 286 hours on the A320 aircraft...

And as for what wound up happening with regard to the crash that would result involving Flight 202?...

Well on takeoff from Karachi at the start of Air Blue Flight 202s flight over to Islamabad. Captain Chaudhry wound up somewhat berating First Officer Ahmed. Saying that if he could not do some of things that he was quizzing him on, 'then what use are you in the cockpit?' Which unfortunately was what wound up having First Officer Ahmed unwilling to speak up, when Air Blue Flight 202 wound up flying past the airport, and wound up doing a CFIT into the nearby Margalla Hills...

The impact killing all 146 passengers and 6 crew members on board instantly...

And now viewers, we will be moving onto the second of the three aviation accidents of this chapter...And for this one, we will be going all the way back, to December 22, 1999...

And on this particular date Korean Air Cargo Flight 8509, which at the time was being operated by a Boeing 747-2B5F... 

And was operating from Stansted Airport in London, to Milan Malpensa Airport in Milan, Italy...

On this particular flight, were three pilots, and a maintenance mechanic who had done some work on Flight 8509 after its arrival into Stansted from Tashkent in Uzbekistan...

The pilot-in-command of Flight 8509 was 57 year old Captain Park Duk-kyu. Who was a former colonel and pilot in the Republic of Korea Air Force and a highly experienced airman, with a total of 13,490 flying hours. With 8,495 of them having been accumulated flying Boeing 747s. The First Officer, was 33 year old Yoon Ki-sik. Who in contrast to Captain Park, was relatively inexperienced, with just 195 hours of flying experience on the 747 and a total of 1,406 flight hours. The Flight Engineer was 38 year old Park Hoon-kyu. And like the captain, the Flight Engineer had a lot of experience flying 747s. With 4,511 out of his 8,301 total flight hours being accrued in them. And finally, the fourth crew member on board Flight 8509 was 45-year-old maintenance mechanic Kim Il-suk...

And as for the maintenance that mechanic Il-suk had done on the aircraft? Well as it happened, this was going to wind up playing a very big role in the resulting crash shortly after takeoff from Stansted...

As just after takeoff from Stansted, as Flight 8509 was making a left turn. The Captains ADI, or Attitude Direction Indicator, wound up remaining stuck in a level position on its display... 

And even though the aircraft was in a left hand turn, this takeoff was unfortunately at night. So as a result, none of the crew had any visual with the ground after takeoff from Stansted...

And sadly after only about a minute in the air, Flight 8509 wound up impacting the ground at a 90 degree left bank. With the nose of the aircraft impacting the ground just after the left wing...

With the impact killing all four crew members on board instantly...

And for those of you who were going to ask as to why the First Officer did not simply try and take control of Flight 8509 in an effort to save it? Well the reason as to why he chose to not say anything or intervene. Was because the Captain had berated him just before Flight 8509 had pushed back from the gate at Stansted Airport...

And now viewers, we will be moving onto the third and final aviation accident of this chapter...And for this one, we will be a bit forward in time, to May 5, 2007...

And on this particular date, Kenya Airways Flight 507. Which was a scheduled international passenger service between Abidjan, Ivory Coast, and Nairobi, Kenya with a stopover in Douala, Cameroon. Which as mentioned was being operated by Kenya Airways. And at the time, the aircraft being used to operate the route, was a Boeing 737-800...

On board Flight 507, were 108 passengers, 4 flight attendants, and 2 pilots...

The Captain of Flight 507, was 52 year old Francis Mbatia Wamwea. Who had logged 8,500 hours on jetliners...

And the First Officer was 23 year old Andrew Wanyoike Kiuru. Who had joined the airline one year before the accident...

And as for the cause of the crash of Kenya Airways Flight 507?

Well like in Air Blue Flight 202 earlier on in this chapter, the Captain of Flight 507 had berated his First Officer earlier on the Flights previous leg into Douala...

And unfortunately as this flight was taking off from Douala in a somewhat serious storm. And this was on top of it being dark outside of the cockpit windows. Which unfortunately in this case, wound up causing the Captain of Flight 507, to become spatially disoriented...

And when you add in that the Auto Pilot, that was supposed to be engaged after takeoff, and the fact that it was not engaged as it should have been...

The Captain thought that the Auto Pilot was flying the plane. When in reality, it had not been engaged...

And as a result, the only time when the Captain realized that the Auto Pilot was not engaged, was when the aircraft's bank angle warning sounded...

And not only did the Captain wind up acting inappropriately to the warning. But he wound up making the situation worse as a result...

As Flight 507 wound up rolling until it was basically inverted at around 110 degrees...

Which after a couple seconds of nose diving, Flight 507 would wind up crashing into a mangrove swamp...

With the impact killing all 114 passengers and crew on board instantly. So the moral of these accidents viewers, is to never treat others like they are nothing...

As when you do not wind up treating others with respect, it can wind up leading to accidents such as these...

And finally viewers, though I have said this in many of these same chapters before...

I will once again reiterate, that I don't mean any sort of disrespect, to those whose lives were lost by either this tragedy, or those like it...

As the only reason why I am mentioning them in these fanfics, is to help preserve the legacy of those who were injured or killed in these tragedies...

And also, to make sure that those who lost people in these tragedies, have some sort of another way to remember their loved ones...
